Is Family Health Choice an insurance plan?
No. Family Health Choice is not insurance. We are a Florida-licensed Prepaid Health Clinic (PHC) and Discount Plan Organization (DPO) that provides members with direct access to healthcare services and discounted rates through participating providers.
What is a Prepaid Health Clinic (PHC)?
A Prepaid Health Clinic is a licensed health plan authorized by the State of Florida to arrange medical services for its members. These plans operate on a managed-care model, connecting members to a trusted network of credentialed providers and specialists to help maintain good health at affordable rates.
What is a Discount Plan Organization (DPO)?
A Discount Plan Organization offers members access to healthcare providers who have agreed to provide services at discounted rates. Members pay a low monthly fee for access and pay providers directly at the reduced rates when they receive care.
What is the Hospital Medical Advocacy Program?
Our Hospital Medical Advocacy Program helps members manage large or unexpected hospital bills. Advocates work directly with hospitals and providers to negotiate lower out-of-pocket costs and may assist with financial assistance applications or settlement offers when applicable.
What is the Easy Drug Card?
The Easy Drug Card is a prescription discount program that helps members save money on medications at participating pharmacies. You can use it at major chains and local pharmacies to access discounted drug prices instantly.
What is COSTCARERx?
This program leverages the power of mail order for chronic / long term medications and encourages the participant to use retail pharmacies for acute care / short term medications. The program includes a total of 800+ medications available at a $0 cost and has medications in most major therapeutic drug categories.
Who can join Family Health Choice?
Everyone is welcome. There are no restrictions based on immigration status, Social Security number, or preexisting conditions. If you live in Florida and want affordable, reliable healthcare access, you qualify.
How do I enroll?

When can I start using my plan?
Your plan becomes active as soon as your transaction is complete. You can schedule and visit a doctor the same day you become active.
Do I need to choose a Primary Care Physician (PCP)?
For Health Plan Members (PHC):
Yes, you must select your Primary Care Physician during enrollment. You can always change your PCP later if you prefer a different provider in the network.
For Discount Plan Members (DPO)
No. Discount Plan Members do not need to select a Primary Care Physician.
Do I need referrals to see a specialist?
No. You have direct access to specialists in your plan. Simply schedule an appointment with your chosen provider — no referral required.
Do Health Plan Members (PHC) have a co-payment?
Yes, but they’re minimal and transparent:
- $15 for Primary Care visits
- $30 for Specialists
- $0 for Telemedicine (in most plans)
Your specific copay depends on your plan type and chosen benefits.
Do Discount Plan Members have an office visit fee?
Yes, but they’re minimal and transparent:
- $40 to $60 for Primary Care visits
- $60 to $120 for Specialists
Are there deductibles or hidden fees?
No. Family Health Choice plans have no deductibles, no claim forms, and no surprise bills. What you see is what you pay.
Are prescriptions included in my plan?
Through CostCareRx, members who choose to include prescriptions in their plan have access to more than 800 medications at $0 cost through participating pharmacies. Additional prescriptions are available at discounted rates with the Easy Drug Card.
Does my plan include dental and vision coverage?
Yes. Most Family Health Choice plans include preventive dental and vision benefits, such as cleanings, exams, and basic eyewear discounts. You can also add enhanced options during enrollment.
How can I find a doctor or medical provider?
Log into your Member Portal and click “Find a Medical Provider.” You’ll be able to search by specialty, location, or provider name within our credentialed network.
How can I get my Member ID card?
Once enrolled, log into your Member Portal to access and download your digital Member ID card. You can print it or show it on your phone when visiting a provider.
How soon can I schedule an appointment?
As soon as your transaction clears. Once your plan is active, you can schedule an appointment the same day with any participating provider.

Can I cancel or change my plan?
