Enroll in Family Health Choice
You can enroll online or through a sales representative. All individuals accepted — no SSN or immigration status needed. No medical history restrictions.
Once your enrollment is complete, you’ll receive:
- A confirmation email with your plan details
- A welcome message from Member Services
- Instructions to access your Member Portal
- Your digital ID card (printable or accessible online anytime)
You can start using your plan as soon as the transaction is complete.
Choose Your Providers
Select your
Primary Care Physician (PCP) and any specialists you want direct access to — no referrals required.
You can choose from Family Health Choice’s network of credentialed medical providers across Florida, including:
- Primary Care Doctors
- Specialists (Cardiology, Gynecology, Endocrinology, etc.)
- Telemedicine
- Dental and Vision Providers
- Mental Health Professionals
- Lab Services
- Diagnostic Imaging
You can search providers anytime in the Member Portal under Find a Doctor.
