Family Health Choice is a Florida Licensed Health Plan with a certificate of authority to operate a Prepaid Health Clinic (PHC) and Discount Plan Organization (DPO). The Prepaid Health Clinic (PHC): Family Health Choice pays for PHC services rendered by participating providers. Members will pay a minimal co-payment when receiving PHC healthcare services. Preventive panels at no cost to members, plan benefits are available based upon eligibility. The Discount Plan Organization (DPO): This plan is not a health insurance policy. This plan provides discounts at certain health care providers for medical services. This plan does not make payments directly to the providers of medical services. The Plan members must pay for all health care services but will receive a negotiated discount rate from participating medical providers.
